Step 3: Mail Within 30 Days*
Mail the completed form to the address noted on the Redemption Form.
*Your Redemption Form must be postmarked within 30 days of your ADT Security system installation date, in order to claim your
Visa Reward Card.
Your Visa gift card is provided by your Authorized ADT Provider and is not sponsored directly by ADT Security Services. Limit one Visa gift
card per household.
